CentraCare is part of the National PACE Association (Program of All-Inclusive Care for the Elderly). We are a medical/social-based model of care that provides comprehensive, integrated managed care to frail older adults who are at risk for nursing home placement. Through interdisciplinary team planning and coordination, older adult participants receive coordinated care as they age in place at home. All services and products covered through Medicare and Medicaid are provided by CentraCare PACE. 

Job Description

Provide direct and indirect nursing care to assigned participants, working under the supervision of the Center Director or designee.  Is independent in the application of advanced nursing knowledge and skills and is able to manage complex clinical situations for which he/she is responsible. 

Assists in facilitating the integration of new participants into CentraCare healthcare delivery system.

Participates in IDT meetings, assists with keeping the plan of care up to date; writes orders

Administers treatments/procedures/medications and other care as prescribed and according to patient care standards.

Monitors participant medications along with demonstrating appropriate administration; demonstrates understanding of age-related characteristics and needs of patients served; explains nursing procedures in appropriate forms; identifies emergency situations and independently initiates advanced therapy as appropriate; acts as coordinator of patient care with other health care personnel and evaluates patient care measures instituted; understands and demonstrates respect for patient rights and utilizes established mechanisms for management of ethical issues in patient care, and participates in the coordination of the participant’s 24-hour delivery of care services.

Willingly makes acute home visits as needed on a regular basis to provide care to participants or give treatments (may include wound care, lab draws, nurse assessments, etc.)

Documents in the participant’s medical record as required by policy. Includes recording patient care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems.

Good time management and organizational skills.

Ability to respond quickly to patient needs and work at a fast pace.

Willingly accepts responsibilities as assigned.

Delegates nursing care to appropriate personnel when appropriate.

 

Qualifications

LPN–Nursing Diploma from accredited School of Nursing with Licensure.

Minimum of one year of experience working with the frail, elderly, or long term care population.

Must possess an unencumbered license to practice as a Licensed Practical Nurse in the State of Michigan.

Must have reliable transportation; current valid Michigan driver’s license preferred.

Possess current CPR certification by AHA or willing and able to obtain as offered by CentraCare.

Immunizations and vaccines must be current or medical documentation must be provided from a medical provider with reason (s) to request waiver.  (Hepatitis B, Td, PPD specifically)

Must be able to read, write, speak, and understand the English language and follow written and oral instructions.

Must demonstrate necessary skills and knowledge as outlined in the position specific competency requirements.

Must be knowledgeable of current nursing practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

Must possess the ability to make independent decisions when circumstances warrant such action.

Must possess the ability to communicate tactfully with personnel, participants, family members, visitors, government agencies/personnel, and the public.

Must have proven leadership experience and skills with the ability to get along harmoniously with others in a team environment.

Must possess the ability to plan, organize, develop, implement, and interpret CentraCare goals, objectives, policies and procedures, etc., that are necessary for providing quality care.

Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult participants.

Must be knowledgeable of computers, data entry, output, system applications, etc.
