Job Description

Contra Costa Interfaith Housing (CCIH) is seeking a Case Manager to provide home-based case management and supportive mental health services for formerly chronically homeless families with special needs who are housed throughout Contra Costa County. Working collaboratively with the Families in Supportive Housing (FISH) Team and Property Management, the Case Manager provides psychosocial, life skills and financial assessments, develops and supports the implementation of family service plans, and facilitates linkage to community and mainstream resources that allow consumers to remain stably housed and achieve the greatest level of self-sufficiency possible.

CCIH is a vibrant and socially responsive non-profit agency which provides permanent housing and vital support services to approximately 1,000 people in Contra Costa County who are homeless or are at-risk for homelessness. Over 35 faith-based communities support our work, although there are no religious requirements or affiliations required to receive services.

Qualifications

Position Qualifications:

  1. Master’s degree or substitution of experience for education as follows:
    1. B.A. Degree and four years’ experience in the human services field (for a total of at least six years, combined with the minimum of two years’ experience indicated below) may substitute for the Master’s degree.
  2. Two years’ experience in the human service field, preferably in dual diagnosis, substance abuse, mental health, and/or homeless setting.
  3. Must be familiar with the needs and issues of homeless adults with mental disabilities. (Mental health or ex-mental health consumers are encouraged to apply).
  4. Spanish speaking a plus.
  5. Outstanding written and verbal communication skills.
  6. Proficiency in the use of Microsoft and database applications.
  7. Must have an operational vehicle, auto insurance, and a valid California driver’s license.
  8. Must pass LiveScan screening and TB test.
  9. Salary contingent upon skill and experience.

Additional Information

This will be a 40 hour per week, non-exempt position; some evening and weekend hours may be required.

  1. As part of the FISH team, assist with assessing families for FISH program eligibility, including completing VI-SPDAT and participating on housing placement teams.
  2. Assist identified applicants with the application process for housing, employing the Housing First approach.
  3. Assist new resident families with move-in logistics, ensuring that they understand their rental agreement and how to reach FISH team members when needed.
  4. Perform comprehensive psychosocial life skills and financial assessments on each new family for which you are assigned.
  5. Working collaboratively with consumers, develop and monitor progress on family service plans, encouraging independent action and/or providing assistance as appropriate.
  6. Assure the provision of all essential services including, but not limited to: mental and physical health care, parenting and youth education support, procurement of entitlements and essential needs (food, clothing, transportation, utilities, school supplies, etc).
  7. Refer for medical, mental health, addiction/recovery, financial, legal, and other community services as needed, and coordinate with all persons/agencies involved in a consumer’s service plan for the duration of service delivery.
  8. Maintain contact with all residents in caseload and provide home visits according to identified level of need.
  9. Participate in team case conferences for entire caseload of FISH Program, and be prepared to provide backup to fellow case manager(s) as needed.
  10. Complete timely and accurate documentation within 24 hours after each service contact.
  11. Assure that timely and accurate data is available to be entered into the Homeless Management Information System (HMIS).
  12. Provide consumers with transportation to essential appointments on an as-needed basis.
  13. Provide a minimum of 5 hours per workday of direct, documented services to individuals and/or groups of consumers.
  14. Attend required meetings including: CCIH staff meetings, FISH team meetings, consumer case conferences, and linkage meetings with other agencies.
  15. Attend regular individual supervision meetings and provide timely reporting on consumer progress.
  16. Represent CCIH in a professional manner in all circumstances.
  17. Maintain awareness of culturally diverse consumer populations and perform duties in a culturally competent manner.
  18. On-call, after hours, and backup work for other FISH Team members may be required.
  19. Comply with policies, procedures, standards of practice, and outcome requirements for the FISH Program.
  20. Comply with all legal/ethical professional guidelines for maintaining consumer confidentiality, protecting consumer rights, advocating on consumers’ behalf, and assuring consumer safety.
  21. Comply with all local, state and federal regulations as well as with CCIH Policies and Procedures.
  22. Complete required personnel-related paperwork and complete expenditure reports for food supplies, petty cash expenses and/or mileage in a timely and accurate manner.
  23. Perform all other duties and responsibilities as assigned.

Physical Requirements:

  1. Ability to walk up and down stairs and up to ½ mile at any one time.
  2. Ability to sit for up to 2 hours without a break.
  3. Ability to perform repetitive movements, such as typing and filing, and the use of commonly used office machines and supplies.
  4. Ability to lift and move up to 25 pounds.
  5. Ability to speak on the telephone for up to 3 hours
  6. Ability to drive an automobile and transport oneself between meetings, including driving at night.
