Job Description

The Community Health Worker (CHW) with the Health Start program provides assistance and support in obtaining health related services for clients of the NATIVE HEALTH, Prenatal Clinic and WIC.  The CHW is a liaison between the Department of Health Services, the health care provider, and the Phoenix Indian Community. This position is under the administrative supervision of the Health Start Program Lead.

Starting Salary: $15.06

KEY ACCOUNTABILITIES:

  1. Deliver services primarily through prescheduled home or office visits and scheduled health related classes.
  2. Identify and enroll pregnant and postpartum women, caregivers, and their children in the Health Start program in accordance with the ADHS policy and procedure manual.
  3. Participates in training identified to be consistent with Health Start objectives to provide education to benefit Health Start participants.
  4. Documents and keeps records of all visits and case management services provided to families.
  5. Supports the organization as needed. 

Qualifications

QUALIFICATIONS/EXPERIENCE/REQUIREMENTS:

  1. CHW must live in the identified neighborhood/community and reflect the characteristics of the community.
  2. High School graduate or GED equivalency.
  3. Has at least three years teaching experience in women’s & children’s health.
  4. A sincere desire to improve the health status of the urban Indian population and NATIVE HEALTH’s community.
  5. Must have a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid AZ driver license during employment. Must be insurable by NATIVE HEALTH liability auto policy.
  6. Must pass a criminal background check with a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment.
  7. Must pass a substance abuse testing upon employment, and submit to a random testing during the course of employment.
  8. Must have an updated Immunization (IZ) Record.
  9. Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an update annually.
  10. Must obtain a CPR Card within ninety (90) calendar days of employment and maintain a valid CPR card during employment.

KNOWLEDGE, SKILLS AND ABLITIES:

  1. Knowledge of the cultural and social background of the Native American and other clients of the NATIVE HEALTH service population.
  2. Ability to meet the performance goals and contractual requirements of the Health Start Program
  3. Ability to drive the NATIVE HEALTH agency or own vehicle to perform job responsibilities.
  4. Ability to relate effectively to clients and families.
  5. Ability to communicate effectively, orally, and in writing with peers and other service organizations
  6. Ability to organize, plan, and work independently.
  7. Ability to maintain confidentiality of client information.
