Since 1976, St. Joseph Center has been meeting the needs of low-income and homeless individuals and families in Venice, Santa Monica, Mar Vista, and surrounding communities.  The Center is a 501(c)(3) nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs.  The Center enjoys broad-based community support as well as a sponsored relationship with its founders, the Sisters of St. Joseph of Carondelet.  St. Joseph Center serves approximately 6,000 individuals annually.

Brief Description of Principal Activities

Multifaceted intervention, prevention, and education services are carried out at four sites on the Westside of Los Angeles.  St. Joseph Center’s integrated programs provide clients with concentrated and coordinated access to services according to the nature of their needs.

St. Joseph Center

Case Manager - Vehicular Homelessness Outreach Program

Full- Time / Non-Exempt

 

St. Joseph Center has been awarded a contract to address vehicular homelessness targeting the Rancho Dominguez area of Los Angeles County. This program will engage residents of the RV’s and assist them in getting permanently housed.

 

Under the supervision of the Director of Operations, the Case Manager works as a member of the Vehicular Homelessness Outreach Program.  The Case Manager will provide the following activities:

 

 

  • Survey the vehicular homeless population utilizing the Vulnerability Index (VI) SPDAT to identify and prioritize this group for permanent housing according to the fragility of their health

 

  • Case Management to meet the needs of each individual
    • Provide a Standardized Assessment
    • Develop Individualized housing plan that addresses housing, health, mental health, and independent living goals

 

  • Mental Health and Substance Abuse – Assessment and Treatment providing
    • Linkages to residential and outpatient services
    • Linkages to the clinician on the team to assess and treat individuals with mental health and/or substance abuse issues

 

  • Rapid Re-Housing working with eligible individuals to secure permanent housing with the use of short-term rental assistance and services so they can transition to full housing stability within 4 months

 

  • Independent Living Skills provision on a one-to-one basis (life skills, budgeting and increasing income)

 

  • Transportation by providing bus tokens and passes, taxi vouchers, and/or direct transportation to clients, as needed

 

  • Benefits, Advocacy, and Assistance with Enrollment by securing eligible entitlements

 

  • Permanent Housing Location Services by assisting at minimum 20 clients in completing the required paperwork, obtaining certification, searching for and identifying appropriate housing, negotiating with landlords, and assisting with the move-in process

 

  • Permanent Housing/Retention Services/Eviction Prevention by making weekly home visits and linkages to needed community-based supportive services

 

  • RV Disposal by assisting in the sale or disposal of the RVs

 

 

 

QUALIFICATIONS:  Bachelors degree or equivalent. Two year’s experience providing homeless services or case management. Familiarity with issues surrounding mental illness and substance abuse. Strong communication skills required including the ability to produce solid documentation in a computer based system. Spanish language skills highly desirable.
